CHILDREN MUST BE 5 yrs OLD BY JULY 31st OF EACH YEAR TO BE ELIGIBLE TO * FOR RETURNING PLAYERS ONLY , must have been on a Fontana Pop Warner team as a Player or Cheerleader the whole 2009 season to qualify, for this sign up.
REQUIRED FOR SIGN-UP'S: 1.Certified Birth Certificate only (NO HOSPITAL OR CHURCH CERTIFICATES WILL BE ACCEPTED AS A PROOF OF BIRTH). 2. Proof Of Residency required, as in a Utility Bill, Gas, Electric, Water only (DRIVERS LICENSE, CABLE BILLS, CELL PHONE BILLS WILL NOT BE ACCEPTED AS PROOF OF RESIDENCY). 3. latest 3rd quarter Report Card of the child required. 4. Registration fee of $185.00 at the sign-up a must. 5. Players and Cheerleaders MUST be present to be signed-up no exceptions. ALL THESE REQUIRMENTS MUST BE MET IN ORDER TO SIGNUP RETURNERS OR REGULAR SIGNUPS, YOU WILL BE TURNED AWAY IF THESE REQUIRMENTS ARE NOT MET! NO EXCEPTIONS TO THESE RULES OR REQUIRMENTS "all dates and times and location are subject to change without notice or notifications".
